Dentistry consent form

This is the form you will complete upon admitting your pet for teeth cleaning or any dental procedure.  There are options available to enhance your pet's comfort and safety.  You may accept or decline those options based on your expectations and budget.  This is just one way that Payson Pet Care customizes the medical treatment for each individual.

Doggie Dental Care

This explains the reasons we care for our dogs' teeth.   It gives a detailed description of what happens "behind the scenes" while your dog is here having his teeth cleaned.  The article further explains what you can do at home to protect your dog's teeth.

Kitten vaccine schedule

This information describes the vaccines kittens need.  It is a reference for you to know when to take your kitten to the vet for shots.

New client / new patient registration form

This is the form you will complete upon arriving for your pet's first visit with us.  It helps us to determine what vaccinations, or special care your pet may need.  Please bring any previous records, vaccination documents, and medications with you as well.
