The building is gray with red brick trim. There is a large sign out front. From Logansport: Take State Road 25 north to State Road 16. Turn right & go approximately 4 miles. The clinic is on the right side of the road just before the town of Twelve Mile. From Peru: Take US 31 north to State Road 16. Turn left & go approximately 6 miles. The clinic is 1/4 mile west of Twelve Mile on the left side of the road. From Rochester: Take State Road 25 south to State Road 16. Turn left & go approximately 4 miles.
